Idiopathic Intracranial Hypertension (IIH): Overview

Idiopathic Intracranial Hypertension (IIH) is a rare disorder which is marked by chronically elevated intracranial pressure (ICP). In this disorder, the pressure inside the skull (that is, the pressure of cerebrospinal fluid which surrounds the brain) increases alarmingly, without any noticeable reason.

Idiopathic Intracranial Hypertension is majorly associated with chronically elevated intracranial pressure, with the most significant neurologic manifestation of the disorder being Papilledema (swelling of the optic disc, that is, the spot of the entry of the optic nerve into the eyeball) which can cause secondary progressive optic atrophy, visual loss, and possible blindness.

The Idiopathic Intracranial Hypertension disorder is also known as Benign Intracranial Hypertension (BIH) and pseudotumor cerebri. However, Idiopathic Intracranial Hypertension is the most commonly used name for describing the disorder. The condition typically occurs without any detectable cause, although it can seemingly be caused by certain conditions which may include: the use of some specific drugs like tetracycline, brain tumor, a blood clot in the brain, or excessive intake of vitamin A.

The risk factors for the potential occurrence of Idiopathic Intracranial Hypertension include obesity (being overweight) or a recent weight gain. With the word ‘Idiopathic’ signifying the unidentified etiology of raised pressure, the Idiopathic Intracranial Hypertension disorder mostly affects obese/overweight women in their reproductive years.

Idiopathic Intracranial Hypertension can occur at any age between 25 and 45 years, though the average age for the occurrence of this disorder is 30 years.

Symptoms of Idiopathic Intracranial Hypertension

dizziness

Some common, non-specific symptoms of Idiopathic Intracranial Hypertension include nausea, dizziness, vomiting, ringing in the ears in time with the heartbeat, brief/sporadic incidents of blindness, shoulder pain, photopsias, and retrobulbar pain. However, more specifically, the symptoms of this disorder are characteristically linked to elevated intracranial pressure (ICP) and Papilledema.

The symptoms of Idiopathic Intracranial Hypertension related to elevated ICP generally include:

  • Non-specific headaches which vary in location, type, and frequency
  • Loss of vision (typically visual field, but hardly ever visual acuity loss)
  • Diplopia (typically horizontal because of nonlocalizing sixth nerve palsy)
  • Radicular pain (normally in the arms)
  • Pulsatile tinnitus.

The symptoms of Idiopathic Intracranial Hypertension associated with Papilledema include some characteristic visual signs which usually include:

  • Transient visual obscurations (chiefly or homogeneously orthostatic)
  • Progressive loss of peripheral vision, either in one eye or in both eyes
  • Blurry vision
  • Distortion of central vision caused by optic neuropathy or macular edema
  • Sudden loss of vision (for example, fulminant IIH).

Treatment of Idiopathic Intracranial Hypertension

The main objective of the treatment methods for the effective management of Intracranial Hypertension is to ensure that the optic nerve function is preserved and the elevated intracranial pressure is controlled. For this purpose, doctors generally advise the use of certain medications which mainly include Acetazolamide and other diuretics (for reducing the production of spinal fluid), along with Primary headache prophylaxis. However, in some cases, doctors may also recommend the use of Corticosteroids or may suggest a surgical procedure for relieving the pressure.
